Once a hive has started to make swarm cells, it will continue in it's pursuit of swarming until acheived. Taking frames from one hive to another will not eliminate swarming in of itself. This allows for 2 queens and perhaps even more effecitvely since the workers can unite.

As little as possible, as much as needed. My customers demand honey early in the season, so I take off what I can in early June. If it is capped it is going to be extracted. I believe that the population has become more informed and perhaps know someone who knows a beekeeper and thus bypasses the Ag center altogether. With the internet availabilty I am also seeing more and more people "searching" before they call the old standby.

This year has been unusaually quiet for me. Sine I get most of the calls that come through the Ag center, I consider it almost concerning that I have had no more than 5 calls all year.

As for queen excluders, it is all about how you wish to mangage your bees. If you don't mind brood in your honey and wax moth destrying your comb, then leave the excluders off.
